Boat Insurance Coverage:
All Members who are listed as a Club Facility, Event or Activity boat driver (“Course Driver”) and/or who will use their boat in a Club Facility (slalom course, event or activity) must maintain active insurance coverage (on their boat) with a minimum of $300,000 Combined Single Limit (“CSL”). Any other boat to be used at a Club Facility, Event or Activity must also have active insurance coverage for that boat with a minimum of $300,000 CSL. Annual confirmation is required by submitting the “declarations page” or equivalent of the insurance policy with this Member Application Form. Failure to maintain minimum boat insurance coverage shall disqualify said boat from all Club Facility, Event and Activity operations.

Application Consent:
As parent/guardian with legal responsibility for a junior participant(s), I am authorized to sign this Application on behalf of the junior participant(s). I consent and agree for the junior participant(s) to be bound by the terms of this Application and I hereby indemnify LSWST for all awards, legal expenses, and settlements arising out of the junior's participation with the Lake Sunapee Water Ski Team and in the use of its slalom course facilities, equipment and premises.

Assumption of Risk:
I understand the act of waterskiing, waterskiing behind a powerful tow-boat, waterskiing in a slalom course and waterskiing in a slalom course located inside State Spar Buoys (depth markers) is an inherently risky and dangerous sport and endeavor and I could become seriously injured or die as a result of participating in this activity. I further understand there may be other risks including but not limited to equipment failures, man-made or natural obstacles and boat driver/observer/passenger actions or negligence that may affect me at any time. I agree to assume any and all risks.

Bylaw and Rule Agreement:
I agree to abide by all provisions of the LSWST Bylaws and Rules and State of New Hampshire Boating Laws. I certify that I am healthy enough for the rigors of slalom water skiing and understand waterskiing is a strenuous sport and one in which it is not unusual for the skier to fall.

Use of Photo Image Consent:
I hereby grants permission to the LSWST to use your or your child(ren)’s image(s), picture or other likeness(es), (collectively, “Image”), whether video, digital or print, for public purposes or otherwise, without restriction as to frequency, duration or medium in connection with these activities. I also understand that, and grant permission to LSWST to list the me and/or my child(ren)’s name(s), which may be posted online or otherwise following any event or activity. I also grant permission to LSWST to utilize my email address for information from the Lake Sunapee Water Ski Team. At any time, you may update your preferences and/or opt out of any e-Mail messages.
